ABSTRACT

Rationale@#Probiotics are live microorganisms that exert beneficial effects on the host, including a reduction of allergic disease symptoms. Lactobacillus reuteri in particular was shown to attenuate the allergic airway, and when used as an adjunct in the treatment of asthma in children, resulted in decreased fractional exhaled nitric oxide and interleukin levels when compared to placebo. However, insufficient information is available regarding the significance of L. reuteri as an adjunct in the treatment of allergic disorders, particularly in allergic airway disease.

ABSTRACT

ABSTRACT Numerous works and clinical research conducted in various institutions have cast fresh light on the notion of trauma and have contributed to the emergence of the concept of early relational trauma. Early relational trauma is a form of trauma rendered complex by the multiplicity of its sources and its modes of expression. Its main features are often attachment and acting-out disturbances. In families where such trauma occurs, links could sometimes be perverted and form a narcissistic configuration. This mode of functioning, organized around negativity, impacts the plurality of psychic processes and interferes with the treatment approaches of multidisciplinary teams. In order to promote a multifocal approach to care tailored to differences in situations, educational and socio-judicial practitioners.


RESUMO Inúmeros trabalhos e pesquisas clínicas realizados em diversas instituições trouxeram à luz a noção de trauma e contribuíram para o surgimento do conceito de trauma relacional precoce. O trauma relacional precoce é uma forma de trauma complexo pela multiplicidade de suas fontes e seus modos de expressão. Suas principais características são frequentemente distúrbios de apego e atuação. Nas famílias onde ocorre tal trauma que às vezes pode ser pervertido em configurações narcísicas, esse modo de funcionamento organizado em torno da negatividade impacta a pluralidade do psíquico e interfere nas abordagens de tratamento das equipes multidisciplinares. Para promover uma abordagem multifocal de cuidados adaptados às diferenças de situações, profissionais de educação e sócio judiciais.

Chronic stress leads to circadian disruption, with variability in sleep time and duration. This scenario increases the prevalence and incidence of cardiometabolic abnormalities. Social jetlag (SJL), a proxy of circadian disruption, has been associated with increased vulnerability to the development of metabolic syndrome, obesity, and type 2 diabetes. This research aimed to evaluate how variables associated with cardiometabolic risk are related to SJL and poor sleep among university professors. From 2018 to 2019, full-time university professors (n=103) with a mean age of 44±5.4 years were assessed for sleep quality, chronotype, SJL, metabolic components, sociodemographic characteristics, and physical evaluation. Sleep quality and weekday sleep duration were associated with stress (r=0.44 and r=-0.34) and anxiety (r=0.40), respectively. Mean sleep duration (n=65) was 7.0±1.1 h and all professors with poor sleep (41.2%; n=28) worked 40 h/week. Professors who slept less were significantly (r=-0.25) older, and teaching time (years) was positively correlated with blood glucose (r=0.42). Mean SJL was 59.8 ±4.5 min (n=68) and 48.5% of these professors had values ≤1 h and 51.4% ≥1 h. SJL and blood glucose concentration were associated (r=0.35), which reinforced that challenges to the circadian system reverberate on metabolism. In this study, professors at the Federal University of Rio Grande do Norte had cardiometabolic risks related to anxiety, stress, and sleep quality.

Níveis elevados de ambivalência estão associados a piores resultados e abandono terapêutico. Este estudo teve como objetivo a adaptação transcultural e avaliação das propriedades psicométricas do Questionário de Ambivalência em Psicoterapia (QAP). Após adaptação cultural do instrumento, foram avaliados 181 indivíduos em psicoterapia de diversas abordagens, em clínicas-escola e consultórios particulares. O QAP foi avaliado quanto à estrutura interna e relação com variáveis externas. Na análise fatorial confirmatória, o modelo com melhor ajuste foi composto por três dimensões: duas de primeira ordem - Desmoralização e Alternância - e uma de segunda ordem - Ambivalência. A consistência interna foi 0,86. Na relação com variáveis externas, o QAP se correlacionou moderadamente com medidas de desconforto psicológico (OQ-45- Outcome Questionnaire) (r = 0,65) e motivação para mudança (URICA - University of Rhode Island Change Assessment): contemplação (r = 0,36) e manutenção (r = 0,37). A versão brasileira do QAP demonstrou ser um instrumento com boas características psicométricas, de aplicação rápida e fácil. (AU)


High levels of ambivalence are associated with poor outcomes and therapeutic abandonment. The objective of this study was to conduct a cross-cultural adaptation and assessment of the psychometric properties of the Ambivalence in Psychotherapy Questionnaire (APQ). After cultural adaptation of the instrument, we assessed 181 individuals undergoing psychotherapy of different approaches in school clinics and private practices. The APQ was validated regarding internal structure and relationship with external variables. In the confirmatory factor analysis, the model with the best fit was composed of three dimensions: two first-order - Demoralization and Wavering - and one second-order - Ambivalence. The internal consistency was 0.86. In relation to external variables, the APQ correlated moderately with measures of psychological distress (OQ-45- Outcome Questionnaire) (r = 0.65) and motivation to change (URICA- University of Rhode Island Change Assessment): contemplation (r = 0.36) and maintenance (r = 0.37). The results indicated that the Brazilian version of the APQ is a suitable instrument, of quick and easy application. (AU)

Después de la adaptación cultural del instrumento, 181 individuos fueron evaluados en psicoterapia utilizando diferentes enfoques, en clínicas-escuelas y consultorios privados. El QAP fue evaluado por su estructura interna y su relación con variables externas. En el análisis factorial confirmatorio, el modelo con mejor ajuste estuvo compuesto por tres dimensiones: dos de primer orden - Desmoralización y Alternancia - y una de segundo orden - Ambivalencia. La consistencia interna fue de 0,86. Resumo Fundamentos A doença do coronavírus 2019 (Covid-19) pode levar à insuficiência respiratória grave e lesão cardíaca aguda, mas não está claro com que frequência ela pode causar disfunção cardíaca. Objetivos Nesta revisão sistemática, nosso objetivo foi resumir os principais achados ecocardiográficos em pacientes com Covid-19. Métodos Conduzimos uma busca sistemática nos bancos de dados PUBMED, EMBASE, LILACS e Cochrane, além de artigos não pulicados ( preprints ) no MedRxiv e Scielo desde o início até 21 de julho de 2021. Foram incluídos estudos que apresentaram dados ecocardiográficos de pacientes com Covid-19. Características demográficas, doença cardiovascular (DCV) prévia, e achados ecocardiográficos foram extraídos dos estudos. Realizamos uma metanálise de proporções para estimar os principais achados ecocardiográficos. O nível de significância foi p<0,05. Resultados Do total de 11 233 estudos, 38 preencheram os critérios de inclusão e foram incluídos na metanálise. A proporção estimada de disfunção sistólica do ventrículo esquerdo (VE) foi 25% (IC95%: 19, 31; I2 93%), strain longitudinal global anormal 34% (IC95% 23, 45; I2 90%), disfunção sistólica do ventrículo direito (VD) 17% (IC95% 13, 21; I2 90%), derrame pericárdico 17% (IC95%: 9, 26; I2 97%), e hipertensão pulmonar 23% (IC95%: 15, 33, I2 96%). Disfunção sistólica do VE foi diretamente associada com prevalência de ecocardiograma anormal prévio nos estudos (p<0,001). A proporção de pacientes em ventilação mecânica, indicando gravidade da doença, não explicou a heterogeneidade nas proporções de disfunção do VE (p=0,37). Conclusão Entre os pacientes internados com Covid-19, a disfunção ventricular esquerda foi descrita em um quarto dos pacientes, com menores proporções de disfunção do ventrículo direito, derrame pericárdico e hipertensão pulmonar. No entanto, houve uma proporção mais alta de disfunção do VE nos estudos que relataram presença de doença cardíaca prévia, sugerindo que a disfunção cardíaca era predominantemente pré-existente.


Abstract Background Coronavirus disease 2019 (Covid-19) can lead to severe respiratory distress and acute cardiac injury, but it is unclear how often it can cause cardiac dysfunction. Objective In this systematic review, we aimed to summarize the main echocardiographic findings in patients with Covid-19. Methods We systematically searched in PUBMED, EMBASE, LILACS and Cochrane databases, in addition MedRxiv and Scielo preprints from inception to July 21st, 2021. Studies reporting echocardiographic data in patients with Covid-19 were included. Demographic characteristics, previous cardiovascular disease (CVD), and echocardiographic findings were extracted. We performed a meta-analysis of proportions to estimate the main echocardiographic findings. The level of significance was p < 0.05. Results From 11,233 studies, 38 fulfilled inclusion criteria and were included in the meta-analysis. The estimated proportions of left ventricular (LV) systolic dysfunction were 25% (95%CI: 19, 31; I293%), abnormal global longitudinal strain 34% (95% CI 23, 45; I290%), righ ventricular (RV) systolic dysfunction 17% (95%CI 13, 21; I290%), pericardial effusion 17% (95%CI: 9, 26; I297%), and pulmonary hypertension 23% (95%CI: 15, 33, I2 96%). LV systolic dysfunction was directly associated with study-specific prevalence of previous abnormal echocardiogram (p<0.001). The proportion of patients in mechanical ventilation, indicating severity of disease, did not explain the heterogeneity in the proportions of LV dysfunction (p=0.37). Conclusion Among hospitalized patients with Covid-19, LV dysfunction has been reported in one quarter, with smaller proportions of right ventricular dysfunction, pericardial effusion and pulmonary hypertension. However, there was a higher proportion of LV dysfunction among studies reporting the presence of prior heart disease, which suggests that cardiac dysfunction was mostly pre-existing.

Background and Objectives@#The effects of home or community-based interventions in increasing physical activity in youth with intellectual disabilities are yet to be known. Such information is important in designing the care for this population because of their increased susceptibility to lifestyle diseases that affect their quality of life. This study aimed to determine the effectiveness of home or community-based interventions in increasing physical activity levels in youth with intellectual disabilities. @*Methodology@#Eight databases were searched to identify intervention studies available in English that investigated the effects of home or community-based interventions on physical activity levels in youth with intellectual disabilities. Data were analyzed using descriptive statistics and meta-analysis using a random effects model. Effect sizes were calculated as standardized mean differences with 95% confidence intervals. @*Results@#Six randomized controlled trials were included in this review. Home or community-based interventions had a small effect in improving physical activity levels in youth with intellectual disabilities. These effects did not reach statistical significance for physical activity levels (d = 0.33; CI = -0.11 to 0.78; p = 0.14), number of steps per unit of time (d = 0.23; CI = -0.23 to 0.68; p = 0.32), and time spent inactive (d = 0.15; CI = -0.29 to 0.60; p = 0.50). @*Conclusion@#Home or community-based interventions may be effective in increasing physical activity levels in youth with intellectual disabilities. Further research with homogeneous outcome measures and larger sample sizes need to be conducted to build on the results of this review.

ABSTRACT

@#Spider venoms and toxins are valuable sources of lead compounds for drug development due to their essential role in cellular and physiological processes targeting various receptors. Here, we present the protein profile of the venom of Phlogiellus bundokalbo, an endemic Philippine tarantula, to screen and characterize its cytotoxicity against MCF-7 cells, secretory phospholipase a2 (sPLA2), and neurotoxicity to evaluate its potential anticancer properties. Spider venom was extracted via electrical stimulation. Venom components were fractionated by reversed-phase high-performance liquid chromatography and characterized through liquid chromatography-mass spectrometry (LC-MS) and SDS-PAGE analysis before assay. The resulting five venom fractions were amphiphilic peptides showing cytotoxicity against MCF-7 cells in a concentrationdependent manner (IC50 ranging from 52.25μg/ml to 110.20μg/ml) after 24-hour incubation. Cells appeared detached, rounded, and shrunk with cytoplasmic condensation upon overnight incubation with venom fractions. The sPLA2 was observed in all the venom fractions tested for cytotoxicity. Venom fractions revealed a predominant mass of ~3-5 kDa with LC-MS analysis. Results showed distinct similar mass as μ- theraphotoxin-Phlo1a, an Australian tarantula, Phlogiellus sp. toxin with inhibitor cystine knot motif. The venom fractions exhibit excitatory neurotoxins that might activate presynaptic voltage-gated ion channels, such as an agonist or gating modifier toxins that slow down the channel inactivation similar to spider toxins. In conclusion, the spider venom of P. bundokalbo exhibits cytotoxic, phospholipase A2, and neuroactive properties suggesting that its venom components, upon further purification and structure-function analysis, can be potential tools in the development of targeted breast chemotherapeutics.

ABSTRACT

Spider venom is a rich cocktail of neuroactive compounds designed to prey capture and defense against predators that act on neuronal membrane proteins, in particular, acetylcholinesterases (AChE) that regulate synaptic transmission through acetylcholine (ACh) hydrolysis - an excitatory neurotransmitter - and beta-secretases (BACE) that primarily cleave amyloid precursor proteins (APP), which are, in turn, relevant in the structural integrity of neurons. The present study provides preliminary evidence on the therapeutic potential of Phlogiellus bundokalbo venom against neurodegenerative diseases. Methods Spider venom was extracted by electrostimulation and fractionated by reverse-phase high-performance liquid chromatography (RP-HPLC) and characterized by matrix-assisted laser desorption ionization-time flight mass spectrometry (MALDI-TOF-MS). Neuroactivity of the whole venom was observed by a neurobehavioral response from Terebrio molitor larvae in vivo and fractions were screened for their inhibitory activities against AChE and BACE in vitro. Results The whole venom from P. bundokalbo demonstrated neuroactivity by inducing excitatory movements from T. molitor for 15 min. Sixteen fractions collected produced diverse mass fragments from MALDI-TOF-MS ranging from 900-4500 Da. Eleven of sixteen fractions demonstrated AChE inhibitory activities with 14.34% (± 2.60e-4) to 62.05% (± 6.40e-5) compared with donepezil which has 86.34% (± 3.90e-5) inhibition (p > 0.05), while none of the fractions were observed to exhibit BACE inhibition. Furthermore, three potent fractions against AChE, F1, F3, and F16 displayed competitive and uncompetitive inhibitions compared to donepezil as the positive control. Conclusion The venom of P. bundokalbo contains compounds that demonstrate neuroactivity and anti-AChE activities in vitro, which could comprise possible therapeutic leads for the development of cholinergic compounds against neurological diseases.(AU)

ABSTRACT

The elastic cartilage is composed by chondroblasts and chondrocytes, extracellular matrix and surrounded by perichondrium. It has a low regeneration capacity and is a challenge in surgical repair. One of obstacles in engineering a structurally sound and long-lasting tissue is selecting the most appropriate scaffold material. One of the techniques for obtaining biomaterials from animal tissues is the decellularization that decreases antigenicity. In this work, alkaline solution was used in bovine ear elastic cartilages to evaluate the decellularization and the architecture of the extracellular matrix. The cartilages were treated in alkaline solution (pH13) for 72 hours and lyophilized to be compared with untreated cartilages by histological analysis (hematoxylin-eosin, Masson's trichrome and Verhoeff slides). Areas of interest for cell counting and elastic fiber quantification were delineated, and the distribution of collagen and elastic fibers and the presence of non-fibrous proteins were observed. The results demonstrated that the alkaline solution caused 90% decellularization in the middle and 13% in the peripheral region, and maintenance of the histological characteristics of the collagen and elastic fibers and non-fibrous protein removal. It was concluded that the alkaline solution was efficient in the decellularization and removal of non-fibrous proteins from the elastic cartilages of the bovine ear.(AU)


A cartilagem elástica é composta por condroblastos e condrócitos, matriz extracelular e envolta por pericôndrio. Possui uma baixa capacidade de regeneração e é um desafio em reparos cirúrgicos. Um dos obstáculos na engenharia de tecido estruturalmente sólido e de longa duração é a seleção do material de arcabouço mais adequado. Uma das técnicas para obtenção de biomateriais oriundos de tecidos animais é a descelularização, que diminui a antigenicidade. Neste trabalho, foi utilizada solução alcalina em cartilagem elástica auricular bovina para avaliar a descelularização e a arquitetura da matriz extracelular. As cartilagens foram tratadas em solução alcalina (pH13) durante 72 horas e liofilizadas, e comparadas com cartilagens não tratadas por análise histológica (hematoxilina-eosina, tricrômio de Masson e Verhoeff). Foram determinadas as áreas de interesse para contagem celular e quantificação de fibras elásticas, observada a distribuição de colágeno e fibras elásticas e a presença de proteínas não fibrosas. Os resultados demonstraram que a solução alcalina causou 90% de descelularização na região central e 13% na região periférica, manutenção das características histológicas do colágeno e fibras elásticas e remoção das proteínas não fibrosas. Concluiu-se que a solução alcalina foi eficiente na descelularização e retirada de proteínas não fibrosas de cartilagens elásticas da orelha de bovinos.(AU)

INTRODUCTION@#Stigmatizing attitudes are barriers to treatment of mental health disorders. The burden of stigma has not been established locally. This study aimed to assess the stigma in the community by determining the knowledge, attitudes and behaviors of barangay residents towards mental health and persons with mental health illness.@*METHODS@#A total of 422 participants were included using convenience sampling. Participants were given self-administered questionnaires that consisted of the Mental Health Knowledge Schedule (MAKS), Community Attitudes Towards the Mentally Ill (CAMI), and Reported and Intended Behavior Scale (RIBS) tools. The mean scores and percentages were computed and compared across the sociodemographic data of the respondents.@*RESULTS@#Knowledge levels were relatively high with a mean score of 26.63. Depression, stress, bipolar disorder and drug addiction were recognized as mental illnesses by the majority of the participants. Scores in the stigmatizing ideologies authoritarianism (3.07) and social restrictiveness (2.58) were low, while the positive ideologies benevolence (3.76) and community health ideology (3.85) had higher scores. Participants were reluctant to work with mentally-ill people (3.18) but were willing to be friends with them (3.87).@*CONCLUSION@#This study concludes that the respondents were generally knowledgeable about mental health illness. There was a general acceptance and less stigmatizing attitude, and a willingness to interact with people with mental illness.

ABSTRACT

Corn silage supplementation for dairy cows grazing in temperate annual pastures has rarely been investigated. The aim of this study is to compare two supplementation levels (0 and 4kg dry matter [DM]/day of a 7:1 mixture of corn silage and soybean meal) in dairy cows strip-grazing annual ryegrass (Lolium multiflorum Lam.) at two pasture allowances (PA, low= 25 and high = 40kg DM/d at ground level). The study was carried out according to an incomplete 4 × 3 Latin square design, using 12 cows and three experimental periods of 12 days. The green leaves allowances were only 4.9 and 8.5kg DM/d at the low and high PA, respectively. The total DM intake and milk production increased in supplemented cows compared to un-supplemented cows at the low PA, but were similar between supplementation levels at the high PA. The PI was unaffected by the PA, whereas the substitution rate was 0.68 in cows at the low PA and 1.35 in cows at the high PA. Corn silage supplementation may improve the total DM intake and milk production of dairy cows grazing in temperate annual pastures, but only at a low PA.(AU)


O objetivo deste estudo foi comparar os efeitos de dois níveis de suplementação (0 e 4kg de matéria seca [MS]/dia de uma mistura de silagem de milho e farelo de soja na razão de 7:1) para vacas leiteiras em pastos de azevém anual (Lolium multiflorum Lam.) manejados em faixas diárias com duas ofertas (OF, baixa= 25 e alta= 40kg de MS/dia medidas em nível do solo). Os tratamentos foram comparados em delineamento quadrado latino incompleto 4 × 3, com 12 vacas em três períodos de 12 dias. A oferta de folhas foi somente de 4,9 e 8,5kg de MS/dia nas OF baixa e alta, respectivamente. O consumo total de MS e a produção de leite aumentaram com a suplementação somente quando os animais estavam em baixa OF, não havendo efeito da suplementação em alta OF. O consumo do pasto não foi afetado pela OF, entretanto a taxa de substituição foi de 0,68 em baixa OF e de 1,35 em alta OF. A suplementação com silagem de milho promove o aumento do consumo total de MS e da produção de leite de vacas manejadas em pastos anuais de clima temperado somente em situações de baixa OF.(AU)

Objective: A large proportion of psychotherapy patients remain untreated, mostly because they drop out. This study compares the short- and long-term outcomes of patients who dropped out of psychotherapy to those of therapy completers. Methods: The sample included 63 patients (23 dropouts and 40 completers) from a controlled clinical trial, which compared narrative therapy vs. cognitive-behavioral therapy for major depressive disorder. Patients were assessed at the eighth session, post-treatment, and at 31-month follow-up. Results: Dropouts improved less than completers by the last session attended, but continued to improve significantly more than completers during the follow-up period. Some dropout patients improved with a small dose of therapy (17% achieved a clinically significant change before abandoning treatment), while others only achieved clinically significant change after a longer period (62% at 31-month follow-up). Conclusion: These results emphasize the importance of dealing effectively with patients at risk of dropping out of therapy.Patients who dropped out also reported improvement of depressive symptoms without therapy, but took much longer to improve than did patients who completed therapy. This might be attributable to natural remission of depression. Further research should use a larger patient database, ideally gathered by meta-analysis.

ABSTRACT

Renal ischemia can be associated with some urological procedures, such as renovascular surgery or kidney transplantation, that are often followed by acute renal failure. The aim of this study was to verify the E-cadherin and ß-catenin localization in canine kidney in different times of renal ischemia and reperfusion after chlorpromazine application. Twelve dogs were randomly distributed equally into two groups. GroupA with ischemia and reperfusion without chlorpromazine and groupB with ischemia and reperfusion treated by chlorpromazine. GroupB received intravenous chlorpromazine, 15 min before the artery obstruction, which lasted 1 hour. After this period, the clamps in the renal arteries were released and the organ remained in reperfusion for 2 hours. In each group, anti-E-cadherin and anti-ß-catenin antibodies were made in six tissue samples from renal parenchyma. E-cadherin and ß-catenin are differentially expressed in segments from cortex and medulla in dog's kidneys and the use of chlorpromazine did not alter the expression of both proteins. Occlusion of the left renal artery in dogs causes morphological alterations mainly in proximal convoluted tubules, beginning 30min after the start of ischemia and being aggravated after two hours of reperfusion. These results reveal that chlorpromazine did not change kidneys' histological aspect nor E-cadherin and ß-catenin expression.(AU)


A lesão renal isquêmica pode estar associada a procedimentos urológicos, tais como cirurgia renovascular, cirurgia renal extracorpórea ou transplante renal. Essa injúria, muitas vezes, é seguida de insuficiência renal aguda. O objetivo deste trabalho foi observar a localização da E-caderina e da ß-catenina em rim de cães, além de relacionar a expressão dessas proteínas das junções de aderência em diferentes tempos de isquemia e reperfusão com ou sem a aplicação de clorpromazina. Para tanto, foram utilizados 12 cães, distribuídos aleatoriamente em dois grupos de seis indivíduos: grupo A, com isquemia e reperfusão sem tratamento por clorpromazina, e o grupo B, com isquemia e reperfusão tratado por clorpromazina. No procedimento cirúrgico, foi realizada uma incisão paracostal esquerda para identificação e isolamento do rim esquerdo e da artéria renal esquerda. Após o isolamento da artéria, os animais de todos os grupos tiveram o vaso ocluído. Os animais do grupo B receberam clorpromazina via endovenosa, na dose de 5mg/kg, 15min antes da clampagem do vaso, que durou uma hora. Após este período, as artérias renais foram desobstruídas e os órgãos permaneceram em reperfusão por duas horas. Em cada grupo, foram extraídas seis amostras de parênquima renal, com utilização de agulha tru-cut, para marcação com anticorpos anti-E-caderina e anti-ß-catenina por meio de imunoistoquímica. E-caderina e ß-catenina são diferencialmente expressas em segmentos do córtex e da medula em rim de cães e o uso da clorpromazina não alterou a expressão das duas proteínas.(AU)

Introduction@#This study investigated the association of selected biopsychosocial factors (i.e., CD4 cell count, self-stigma, and social stigma) with the quality of life and adherence to treatment of people living with HIV in the National Capital Region.@*Methods@#A cross-sectional study design was conducted to document the health status and behavior of respondents affiliated with a clinic in Quezon City. Participants answered an online questionnaire containing the Berger HIV Stigma Scale, WHO-QOL for HIV, and HIV Treatment Adherence SelfEfficacy Scale. Bivariate analyses and prevalence risk ratios were used to determine the association of selected biopsychosocial factors with quality of life and adherence to treatment.@*Results@#One hundred respondents were analyzed, of which 42% had CD4 cell counts < 350 cells/mm3, 43% had high self-stigma and 36% had high social stigma while 11% had poor QOL and 7% had poor ATT. There was no significant association of CD4 cell count, self-stigma and social stigma with quality of life and with adherence to treatment.@*Conclusion@#A weak association was noted between poor QOL and low CD4 cell counts and among those who felt higher social stigma, but the relationships were not significant. The association between poor ATT and the selected biopsychosocial factors was not significant.

ABSTRACT Objective To analyze cases of recurrent glioblastoma subjected to reoperation at a Brazilian public healthcare service. Methods A total of 39 patients subjected to reoperation for recurrent glioblastoma at the Department of Neurosurgery, São Paulo Hospital, Federal University of São Paulo, from January 2000 to December 2013 were retrospectively analyzed. Results The median overall survival was 20 months (95% confidence interval – CI = 14.9–25.2), and the median survival after reoperation was 9.1 months (95%CI: 2.8–15.4). The performance of adjuvant treatment after the first operation was the single factor associated with overall survival on multivariate analysis (relative risk – RR = 0.3; 95%CI = 0.2–0.7); p = 0.005). Conclusion The length of survival of patients subjected to reoperation for glioblastoma at a Brazilian public healthcare service was similar to the length reported in the literature. Reoperation should be considered as a therapeutic option for selected patients.


RESUMO Objetivo Analisar o papel da reoperação em pacientes com glioblastoma recidivado em um serviço público no Brasil. Métodos Foram analisados retrospectivamente 39 pacientes submetidos à reoperação por recorrência de glioblastoma no Departamento de Neurocirurgia da Universidade Federal de São Paulo, no período de janeiro de 2000 até dezembro de 2013. Resultados A sobrevida global mediana foi de 20 meses (IC 95% = 14.9–25.2), e a sobrevida mediana após a reoperação foi de 9.1 meses (IC 95% = 2.8–15.4). A realização de tratamento adjuvante após a primeira cirurgia foi o único fator associado com a sobrevida global numa análise multivariada (RR = 0.3; IC 95% = 0.2–0.7; p = 0.005). Conclusão A sobrevida dos pacientes submetidos à reoperação em um serviço público no Brasil é semelhante à reportada pela literatura. A reoperação deve ser considerada como uma opção terapêutica em pacientes selecionados.

ABSTRACT

Resumo: Os artigos apresentados representam diferentes aplicações da teoria do self dialógico. Partindo deste conjunto de contributos teórico-empíricos, desenvolvemos uma reflexão em torno da metáfora da multivocalidade identitária centrada em duas dimensões complementares: a necessidade de uniformização e transversalidade e a necessidade de inovação e diversidade. Nesse sentido, apresentamos uma revisão conceptual de três conceitos-chave desta perspectiva teórica (posição de identidade, voz e posicionamento) e uma análise das interligações com a perspectiva semiótica, a perspectiva cultural e a perspectiva desenvolvimental.


Abstract: These articles represent different applications of the dialogical self theory. From this set of theoretical and empirical contributions, we have developed a reflection on the metaphor of identity multivoicedness. We focused on two complementary dimensions: the need for standardization and transversality and the need for innovation and diversity. In this sense, we present a conceptual review of three key concepts of this theoretical perspective (I-position, voice and positioning) and an analysis of the interconnections with the semiotic perspective, the cultural perspective and the developmental perspective.


Résumé: Les articles présentés représentent différentes applications de la théorie du self dialogique. À partir de cet ensemble de contributions théoriques et empiriques, nous développons une réflexion autour de la métaphore de la multivocalité identitaire centrée sur deux dimensions complémentaires: la nécessité de la normalisation et de la transversalité, et la nécessité de l'innovation et de la diversité. Ainsi, nous présentons une révision conceptuelle de trois concepts clés de cette perspective théorique (position de l'identité, voix et positionnement) et une analyse des interconnexions avec la perspective sémiotique, la perspective culturelle et la perspective développementale.

Children and adolescents under the responsibility of the youth protection have experienced complex traumas concerning neglect, mistreatment, and various forms of abuse. ABSTRACT

El trombohematoma subcoriónico masivo o Mola de Breus, por ser éste el autor que lo describió por primera vez en abortos, es un hematoma subcorial de al menos 1 cm de grosor que ocupa al menos, el 50% de la superficie fetal. Suele asociar una alta morbimortalidad fetal y perinatal, en forma de abortos, retraso del crecimiento intrauterino, muerte intrauterina, desprendimiento de placenta normalmente inserta y recién nacidos de bajo peso, generalmente pretérminos. Esta entidad debe diferenciarse de otros procesos que pueden asentar bajo la placa corial, hacia la superficie fetal como: corioangioma, depósitos de fibrina y trombosis del espacio intervelloso. Se han intentado postular mecanismos que pudieran causar dicha circunstancia, sin que por el momento podamos definir factores de riesgo médicos que claramente predispongan a padecer esta entidad. El objetivo de describir estos dos casos clínicos diagnosticados en 2013 es resaltar que, gracias a la sospecha ecográfica de un trombohematoma subcoriónico masivo, podemos estudiar la pieza de placenta y membranas tras la finalización de la gestación, con lo que obtendríamos así la confirmación anátomo-patológica, de otra manera esta información podría perderse.


The massive subchorionictrombo hematoma or Breus's Mola, this was the author who first described it in miscarriages, is a subchorionic hematoma at least 1 cm in thickness which occupies at least 50% of the fetal surface. Usually associated with high fetal and perinatal morbidity and mortality, as miscarriages, intrau-terine growth retardation, stillbirth, placental abruption and low birth weight, preterm generally. This entity should be distinguished from other processes that can seat under the chorionic plate to the fetal surface as chorioangioma, fibrin deposition and thrombosis intervillous space. They have tried to apply mechanisms that could cause such a circumstance, but for now we don't define medical risk factors that predispose clearly suffer from this entity. In order to describe these two cases diagnosed in 2013 is to emphasize that thanks to the sonographic suspicion of a massive subchorionic trombohematoma, can study the piece of placenta and membranes after the end of pregnancy, so it would provide pathologic confirmation, otherwise this information could be lost.
